Preferred Provider Organization (PPO)
This section provides a summary of the Preferred Provider Organization Plan (the "Plan") as of January 1, 2012.
This section, together with the Administrative Information section and the applicable section about participation, forms the Summary Plan Description and plan document of the Plan.
The Preferred Provider Organization (PPO) offers comprehensive health services from participating and non-participating providers. Generally, your care is covered after you pay a
deductible.
You may select any participating provider in the network to manage your care, or you may choose a non-participating provider. Generally, your costs are lower if you use a participating provider.
Generally, the Plan begins paying benefits for covered care after you pay a deductible.
