Personal Accident Insurance Plan
This section provides a summary of the Personal Accident Insurance Plan as of January 1, 2012.
This section, together with the
Administrative Information section and the applicable section about participation, forms the Summary Plan Description and plan document of the Plan.
The Personal Accident Insurance Plan provides a benefit to someone you name as your
beneficiary if you die in an accident, or to you if you suffer
dismemberment as a result of an accident. Additional coverage is available for your family members (in which case the benefit is payable to you).
The Employee Retirement Income Security Act of 1974 (ERISA) is a federal law that governs many employer-sponsored plans including this one. Your ERISA rights in connection with this Plan are detailed in the
